This community needs assessment, prepared for Axis Health System, provides a data-driven and community-informed analysis of behavioral health needs in La Plata County, Colorado. Using a mixed-methods approach—including quantitative data analysis, stakeholder interviews, and community feedback—the report identifies key trends, service gaps, and barriers to care, particularly in a rural context. Findings highlight both progress and persistent challenges, including workforce shortages, limited access to higher levels of care, and the impact of social determinants such as housing instability. The assessment offers actionable insights to guide service delivery, strengthen care coordination, and support ongoing improvement under the Certified Community Behavioral Health Clinics (CCBHC) model.
